POA status is determined in relation to the time of inpatient admission, not the patient's physical arrival at the hospital. Therefore, a condition that develops during outpatient surgery, emergency-department care, or observation and remains present when the inpatient admission order occurs is generally considered present on admission for inpatient POA reporting.

A clinical validation query is appropriate because the documented diagnosis appears inconsistent with the available clinical evidence. The creatinine values remain essentially unchanged, urine output is normal, and there is no apparent treatment or monitoring consistent with acute renal dysfunction.

The CDI specialist cannot simply delete the diagnosis because providers are responsible for establishing diagnoses. Conversely, a diagnosis should not automatically be accepted when substantial clinical evidence raises a legitimate question about its validity.

The most meaningful indicator is an increased volume of appropriately supported cases grouping to respiratory infection DRGs. The stated purpose of the pilot is not merely to obtain more physician responses; it is specifically to improve documentation of the type or etiology of pneumonia.

When providers document clinically supported pneumonia specificity—for example, an identified bacterial etiology or another qualifying respiratory infection—the coded case may move from a less specific/simple pneumonia grouping into a respiratory infections and inflammations DRG family, depending on the final diagnoses and grouper logic. A robust CDI analysis should also verify that any DRG shift reflects clinically valid documentation rather than query pressure or reimbursement-oriented behavior.
